Indeed, you've come to the right conclusion. The 'choosing between the two' refers to him either (1)making the world a happier place by eradicating his own kinds' existence, i.e. his sibling dragons, as you rightly put it, or (2) surviving by feeding on mankind itself and encouraging his 'siblings' to survive, so as not to fall into extinction. ^_^ I'm glad you were able to read so much into it.
